Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully. MedPAC Rx is required by law to maintain the privacy of your protected health information (PHI), to provide you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

We may use and disclose your PHI without your authorization for treatment, payment, and healthcare operations. For treatment, your information is shared among the providers involved in your care and with the licensed pharmacy that fills your prescription. For payment, we use your information to bill and process transactions. For operations, we use it to run and improve our practice, such as quality review and care coordination.

Other uses and disclosures require your written authorization, including most uses of your PHI for marketing and any sale of your PHI. You may revoke an authorization at any time in writing, except to the extent we have already acted on it. We may also disclose PHI when the law requires it, such as for public health activities or in response to a lawful order.

You have rights over your health information. You may request a copy of your records, ask us to amend information you believe is incorrect, request an accounting of certain disclosures, ask for restrictions on how we use or share your PHI, request that we communicate with you in a certain way or at a certain location, and receive a paper copy of this notice. We will notify you as required by law if a breach affects your unsecured PHI.

If you believe your privacy rights have been violated, you may file a complaint with us at privacy@medpacrx.com or with the U.S. Department of Health and Human Services, Office for Civil Rights.
