Telehealth Informed Consent
Telehealth means receiving medical care remotely. At MedPAC Rx, your provider evaluates you through your online intake and a live consultation by video or phone, and follows your progress through secure messaging and check-ins. Your provider is a licensed medical provider practicing in accordance with Nevada licensing and supervision requirements.
Telehealth has benefits: you can be evaluated and monitored without traveling to a clinic, and you can reach your care team through secure messaging. It also has limits. Your provider cannot perform a hands-on physical exam, technology can fail or interrupt a visit, and in some situations remote care is not enough. If your provider determines that telehealth is not appropriate for your needs, they will tell you and recommend in-person care.
Your participation is voluntary. You may decline or stop telehealth services at any time, ask questions before agreeing to any treatment, and withdraw this consent without penalty. Your provider likewise may determine that a requested treatment is not clinically appropriate; no prescription is guaranteed.
Telehealth is not for emergencies. If you experience a medical emergency, including severe symptoms or a reaction to a medication, call 911 or go to the nearest emergency room. Do not wait for a response through our platform.
Your privacy is protected. Consultations take place over secure technology, and your health information is handled under HIPAA and our Notice of Privacy Practices. As with any electronic system, a small risk of technical failure or unauthorized access exists despite safeguards.
Note to site owner: this consent is a starter template. Have it reviewed by qualified legal counsel and your supervising physician before use, and confirm it meets current Nevada telehealth consent requirements.
